We are seeking a skilled Occupational Therapist to join our Home Health team. As a key member of our care team, you will work closely with patients to develop and implement personalized rehabilitation plans.
Key Responsibilities- Assess and screen patients' daily living and work-related skills to develop effective therapeutic retraining programs.
- Administer and interpret diagnostic and prognostic tests to assist physicians in evaluating patient function.
- Collaborate with healthcare team members to develop and revise patient care plans.
- Provide therapeutic treatment and instruction to patients to improve strength, coordination, and range-of-motion.
- Review and expand retraining programs to meet patient goals and rehabilitation program objectives.
- Consult with vocational team members to develop vocationally oriented plans.
- Report patient status and progress to physicians, rehabilitation staff, and family members.
- Participate in care coordination and discharge planning activities.
- Master's degree from an accredited Occupational Therapy Program.
- Minimum of six months of occupational therapy experience preferred.
- Home Health experience a plus.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
- Valid driver's license, auto insurance, and reliable transportation.
