Job Title: LPN LTC Nurse

We are seeking a skilled LPN LTC Nurse to join our team at a prestigious hospital in Brooklyn, NY.

Job Summary:

The LPN LTC Nurse will be responsible for providing high-quality nursing care to residents in a long-term care setting. This includes performing tasks and responsibilities within the framework of case finding, provision of supportive and restorative care under the direction of a Registered Professional Nurse.

Key Responsibilities:
  • Provides direct nursing care to assigned residents with complex needs, including hygiene and physical comfort, medical treatment, nutritional needs, ambulation and exercise, and elimination maintenance.
  • Directs the care of residents performed by Certified Nursing Assistants.
  • Administers medication to assigned residents, excluding blood and blood products, hanging the first IV solution containing medications, administering IV (push) medication, and administering IV medications via central lines and maintenance.
  • Administers IV therapy, including venipuncture for the purpose of placing a heparin lock or administering IV fluids, setting up and assisting in starting IV fluids, hanging fluids and monitoring fluid rate as presented by physician order, replacing bags of IV fluids, changing intravenous dressing, tubing on peripheral lines, and discontinuing IV fluids.
  • Prepares residents for diagnostic tests, operative procedures, medical regimen examinations, and transportation.
  • Admits residents according to facility and unit policies and procedures and assists them in adjusting to the environment.
  • Participates in the Person Centered Care planning and coordinating of the nursing care in an assigned unit and implements plan.
  • Assists and/or contributes to the development and updating of the Person Centered Care Plan to reflect the residents' needs.
  • Assists RN with health teaching for residents and families as needed.
  • Establishes positive relationships with residents and families in accordance with Residents' Bill of Rights.
  • Participates in unit/team ICP meetings and PI teams.
  • Observes nutritional needs of residents and actively participates at mealtime by assigning staff, feeding residents, and monitoring intake of residents' consumption.
  • Assists in post-mortem care of deceased residents as needed.
Requirements:
  • Demonstrates competence and experience in nursing skills within the scope of practice.
  • Has the ability to establish positive team morale and good interpersonal relationships with residents, families, and co-workers.
  • 1+ years of relevant experience preferred.
  • Completion of LPN program required.
  • Licensed and currently registered as a Practical Nurse in New York State.
  • BLS (American Heart Assoc.) or NY State CPR certification.
