As a Practical Nursing Specialist, you will play a crucial role in fostering a healthy, safe, and supportive environment for the individuals under our care. Your primary responsibility will be to implement and follow through on personalized care plans tailored to each individual’s needs.
Key Responsibilities:
- Engage actively in the planning and execution of daily care activities.
- Provide updates to the Medical Program Coordinator regarding any health-related concerns of the individuals.
- Educate staff members to enhance their understanding of the health conditions of those in our care.
- Conduct health assessments and document findings accurately during each shift.
- Assist with quality assurance checks related to medical procedures.
- Contribute to the Individual Support Plan (ISP) as needed.
- Participate in ISP and Circle of Support meetings as required.
- Monitor and report seizure activity to the RN Healthcare Coordinator.
- Act as a liaison for individuals, aiding in nursing decisions and healthcare coordination.
- Maintain strict adherence to infection control protocols.
- Communicate effectively with physicians regarding health issues.
- Accompany individuals to medical appointments, ensuring all necessary information is communicated.
- Administer medications and treatments following established protocols.
- Review and ensure the accuracy of health-related documentation.
- Compile and submit incident and medication error reports as necessary.
- Maintain organized health records for individuals served.
- Assist in the procurement of medical supplies as needed.
- Ensure compliance with state and federal regulations related to nursing practices.
- Report any concerns regarding client welfare promptly.
- Document vital signs and other health metrics as required.
Qualifications:
- Graduation from an accredited Licensed Practical Nursing Program.
- A minimum of three years of relevant clinical experience, particularly with elderly or intellectually disabled populations.
- Proficient in nursing procedures, first aid, and basic nursing principles.
- Knowledge of intellectual and developmental disabilities, including seizure disorders and Autism.
Physical and Employment Requirements:
- Ability to travel frequently and assist individuals physically.
- Must possess a valid driving record and reliable transportation.
- Willingness to work flexible hours, including evenings as needed.
Working Environment:
This role may require working in diverse settings, both indoors and outdoors, ensuring adaptability and resilience in various conditions.
