Nurse Practitioner Hospice Care Specialist

We are seeking a skilled Nurse Practitioner to join our team at Geisinger Hospice, a part of LHC Group. As a Hospice Nurse Practitioner, you will provide consultation in palliative care, symptom management, and supportive care to meet the needs of our patients.
Responsibilities- Provide face-to-face encounter visits to qualifying Hospice patients and report clinical findings to the Hospice Medical Director.
- Consult with the Hospice Medical Director as needed and inform primary physicians of recommended services.
- Initiate and coordinate the plan of care, documenting problems, goals, interventions, and patient/family response to hospice care.
- Collaborate with patients, families, attending physicians, and other members of the IDG to provide patient and family care.
- Maintain accurate and relevant clinical notes regarding patient condition and report patient condition and identified needs/issues to Administrator and Hospice Medical Director.
- Minimum of five years nursing experience, with clinical nursing care in oncology, hospice, geriatrics, or intensive care preferred.
- Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
- Current certification by a national body such as ANCC, NBCHPN, or AANP.
- Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation.
- Current CPR certification required.
