We are seeking a skilled and compassionate Pediatric Private Duty LPN to join our team. As a Private Duty LPN, you will provide high-quality, one-on-one care to pediatric patients in their homes, working closely with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities
  • Provide direct nursing care to pediatric patients in their homes, following the plan of care and in coordination with other healthcare team members.
  • Document patient visits thoroughly and completely, adhering to regulatory and payer requirements.
  • Participate in our care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, and other clinicians.
  • Ensure confidentiality of patient health information, adhering to agency policy.
Requirements

To be successful in this role, you will need:

  • A current LPN licensure in the state of Tennessee.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
  • Previous home care experience is preferred.
