Job Summary

As a Home Health Nurse Coordinator with King's Daughters, you will play a vital role in providing high-quality patient care in the comfort of their own homes. This position requires a skilled and compassionate Registered Nurse to coordinate total nursing care, participate in patient and family teaching, and lead the care team in maintaining standards for professional nursing practice.

Key Responsibilities

  • Assess patient needs and develop individualized plans of care to meet their physical, psychological, social, and rehabilitative needs.
  • Initiate and update plans of treatment to achieve specific and measurable goals for each patient.
  • Perform and document prescribed treatments, utilizing safe medication practices and adhering to regulatory guidelines.
  • Coordinate and delegate care in accordance with patient needs, capabilities, and qualifications, collaborating with the Home Health team and physicians to improve patient health and well-being.
  • Submit required documentation in a timely manner, adhering to regulatory and agency guidelines.

Requirements

  • Graduate from an accredited school of nursing for Registered Nurses.
  • Maintains active RN license in the Commonwealth of Kentucky (or other eNLC state with multistate privileges).
  • Achieves and maintains American Heart Association Basic Life Support (BLS).
  • Minimum 2 years clinical nursing experience.
  • Current driver's license and daily access to a reliable vehicle.

Preferred Qualifications

  • Bachelor's Degree in Nursing.

Working Environment

This role requires constant exposure to blood and/or body fluids and infection, occasional exposure to toxic or caustic chemicals, and occasional exposure to risk of radiation. The noise level in the work environment is usually moderate, and the role requires constant operation of a motor vehicle long distances in urban and rural areas.

Physical Demands

This position requires occasional lifting and/or moving items up to 100 pounds, constant communication with patients and other care team members, and specific vision abilities including close vision, distance vision, color vision, peripheral vision, and depth perception.
