Job Title: Licensed Practical Nurse

Job Summary:

The Licensed Practical Nurse (LPN) is a vital member of our Assertive Community Treatment (ACT) or Family Practice team, providing person-centered services to address the diverse needs of our beneficiaries, helping them achieve their personal goals.

Key Responsibilities:

  • Collaborate with the psychiatrist to manage the medication system, administer, and document medication treatment.
  • Build rapport and engagement with beneficiaries, promoting a positive and supportive environment.
  • Engage in health promotion, prevention, and education activities, empowering beneficiaries to take control of their health.
  • Develop strategies to maximize medication adherence, working closely with beneficiaries and psychiatric care providers.
  • Communicate and coordinate services with other medical providers, ensuring seamless care delivery.
  • Educate the team on monitoring psychiatric symptoms and medication side-effects, promoting a culture of safety and awareness.
  • Provide crisis response to beneficiaries experiencing a crisis, offering support and guidance during challenging times.
  • Participate actively in daily ACT team meetings, contributing to the development of effective care plans.
  • Prepare and administer oral medications, as well as injectables as prescribed, ensuring accurate and timely delivery.
  • Collaborate with the supervising RN and ACT staff to complete tasks within the proper scope of practice, promoting a culture of teamwork and collaboration.
  • Accurately document all billable encounters into Southeastern Integrated Care's EMR system within 24 hours, ensuring timely and accurate record-keeping.

Qualifications:

To succeed in this role, the ideal candidate will possess:

  • Minimum one year of experience working with adults with serious mental illness.
  • Diploma in Licensed Practical Nursing (LPN).

Required Skills and Abilities:

  • Working knowledge of psychiatric medications.
  • Ability to empathetically interact with beneficiaries and their families.
  • Strong clinical skills, with the ability to operate independently or with minimal supervision.

Certificates, Licenses, and Registrations:

  • Valid NC Nursing License or Compact License that allows practice in NC.
  • Copy of a valid, unrestricted NC Driver's License, proof of insurance.

Work Environment:

The work environment for this role may involve exposure to cigarette smoke, domestic animals/pets, household pests, uncomfortable heating/cooling, and other issues related to the domestic location in which service is delivered.

Physical Demands:

The physical demands of this role include standing, walking, using hands to finger, handle, or feel objects, tools, or controls, talking, and hearing. The employee must be able to lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds. Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. The employee must be able to drive and operate a personal vehicle safely and adhere to all applicable state and traffic laws.
