Job Summary
We are seeking a highly motivated and organized LPN to provide one-to-one care to pediatric patients in their homes. This role involves developing and implementing personalized care plans, working closely with our multidisciplinary team, and communicating effectively with patients and families.

Key Responsibilities
- Provide one-to-one care to pediatric patients in their homes
- Develop and implement personalized care plans that meet the unique needs of each patient
- Work closely with our multidisciplinary team to ensure seamless care delivery
- Communicate effectively with patients and families to address their concerns and needs

Requirements and Qualifications
- Unencumbered license as a Licensed Practical Nurse (LPN) in the state where care is provided
- Minimum of 1 year nursing experience
- Valid Driver's License, Auto Insurance, BLS CPR card, and an up-to-date TB skin test or Chest X-Ray
