About the Role

We are seeking a skilled Medical Professional to join our team as a Physical Therapist in Palos Hills, Illinois. This role involves providing patient care and rehabilitation services to individuals in need of physical therapy.

Key Responsibilities
  • Interpret physician referrals and complete comprehensive evaluations to determine individual treatment plans.
  • Develop and implement personalized treatment plans based on evaluation results and functional goals.
  • Monitor and evaluate patient outcomes, adjusting treatment plans as necessary to ensure optimal results.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Maintain accurate and timely documentation of patient progress, including progress notes, team conference reports, and discharge summaries.
Requirements
  • Degree in Physical Therapy from an accredited institution.
  • Licensed pending physical therapist with authorization letter from IDFPR.
  • Basic Life Support (BLS) certification within 90 days.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to lift up to 50 lbs. and perform repetitive tasks throughout the workday.
Compensation and Benefits
