Hospice Care Nurse Specialist

We are seeking a compassionate and skilled Registered Nurse to join our team as a Weekend On-Call RN. This role involves evaluating patients, creating care plans, and communicating with the patient's family and care team.
Key Responsibilities- Evaluate patients and create personalized care plans
- Communicate effectively with patients, families, and the care team
- Work collaboratively with the 24/7 365 RN Staffed Triage Department

To be successful in this role, you will need:
- A current state license as a registered nurse
- At least 2+ years of nursing experience in a clinical care setting (hospice experience preferred)
- Reliable transportation
- The ability to sit, stand, bend, move intermittently and lift at least 25lbs
