In this role, you will provide high-quality patient care and support to individuals and their families in the comfort of their own homes.

Key Responsibilities
  • Provide direct patient care services, including administering medications, wound care, and other treatments as prescribed by the physician.
  • Evaluate patient needs and develop individualized care plans to ensure optimal comfort and quality of life.
  • Collaborate with the interdisciplinary team, including physicians, nurses, and social workers, to coordinate patient care and address any concerns or issues that may arise.
  • Document patient progress and communicate effectively with the healthcare team to ensure seamless care transitions.
  • Attend staff meetings and participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements
  • Licensed Practical Nurse (LPN) license to practice in the state of employment.
  • 1+ years of experience in hospice, home health, ICU, geriatric, or oncology care.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
