Key Responsibilities:- Conduct patient interviews and physical evaluations to assess and treat new and existing patients.
- Order and perform laboratory tests, diagnostic procedures, and therapeutic interventions.
- Collect and interpret diagnostic data to diagnose medical problems, develop care plans, and prescribe treatments.
- Collaborate with other healthcare professionals to provide comprehensive patient care.
- Provide education and support to patients and their care partners on health habits, self-examinations, and behavior management.
- Document patient histories, assessments, interventions, and interactions using electronic health records.
- Communicate effectively with patients, care partners, and healthcare teams to ensure seamless care coordination.
- Graduate of an accredited Nurse Practitioner or Physician's Assistant program.
- Active certification as a Nurse Practitioner or Physician's Assistant.
- Active NJ licensure as a Nurse Practitioner or Physician's Assistant.
- Basic Life Support certification.
- Advanced Cardiac Life Support certification.
