Director of Social Work (Full Time)
Greenbrook Manor Care and Rehabilitation LLC is in search of a dedicated Director of Social Work. In this pivotal role, you will be responsible for delivering medically-related social services aimed at achieving and sustaining the highest possible physical, mental, and psycho-social well-being of our residents.
Key Responsibilities:
- Design and execute comprehensive case management services for all residents.
- Maintain communication with family members (with resident's consent) to provide updates on health status, current objectives, discharge planning, and encourage involvement in care planning.
- Document progress notes in accordance with federal and state regulations for Long Term Care.
- Promote the development of relationships between residents and staff by training staff to understand and address the unique needs of each resident.
Qualifications:
- Must possess an LCSW or CSW license as a qualified social worker.
- A Bachelor’s degree from an accredited Social Work program is required.
- Experience in subacute and geriatric care settings.
- A minimum of one year of experience working with senior populations in a supervisory role within a hospital, skilled nursing facility, or similar healthcare environment.
