Nurse Health Educator Job Summary

We are seeking a compassionate and dedicated Nurse Health Educator to join our dynamic team at the EAST Haven Crisis Stabilization Center. As a key member of our interdisciplinary team, you will provide essential health education and support to individuals during critical times, empowering them to navigate their crises with resilience and informed care.

Key Responsibilities:
  • Provide preventative health and health literacy education in individual, family, and group modalities.
  • Collect vital signs, including height, weight, blood pressure, temperature, and pulse oximetry.
  • Conduct screenings, such as COWS, Point of Care BAC Testing, AUDIT-C, CIWA-AR, Fagerstrom Test for Nicotine Dependence, and other screenings as indicated and assigned.
  • Administer injections, collect urine samples, and draw labs, when needed.
  • Work in consultation with the EAST Haven counseling staff to triage a higher level of care for individuals experiencing a crisis presentation, if needed.
  • Consult with the staff psychiatrist responsible for psychiatric and medication evaluation when needed.
  • Document encounters with individuals served, including a narrative about their presenting concern, any pertinent medical history, and recommendations for treatment and linkage.
  • Complete all required trainings in a timely manner, as assigned.
  • Maintain knowledge of community resources and assist with linkage and advocacy.
  • Maintain cultural humility and competence with respect to individuals served and their families, as well as other employees and suppliers.
Requirements:
  • A bachelor's degree in nursing, currently licensed as a Registered Nurse by the New York State Department of Education.
  • Additional experience working and/or training in behavioral health is preferred but not required.

Salary starting at $38.00 - 41.00/hour. Flexible scheduling. Up to 30 hours per week.
