Job Summary

We're seeking a skilled and compassionate Long Term Care Coordinator to join our team. In this role, you'll work closely with healthcare providers, members, and their families to develop and implement personalized care plans that promote quality, cost-effective outcomes.

Key Responsibilities
  • Assess member needs and develop holistic care plans that address physical, emotional, and social well-being
  • Collaborate with healthcare providers and partners to ensure seamless care coordination
  • Provide education and support to members and their families on healthcare benefits, services, and resources
  • Monitor care plans and outcomes, making recommendations for improvement as needed
  • Perform home and site visits to assess member needs and collaborate with healthcare providers
Requirements
