Job Title: Medically Skilled Care Provider

About the Role: We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Medically Skilled Care Provider. In this role, you will provide high-quality clinical services to patients in various settings, delivering exceptional patient care within the framework of our organizational philosophy, policy, and standards of community health practice.

Key Responsibilities:

  • Provide skilled nursing care to patients in various settings, including assessment, teaching, treatment, documentation, and care coordination.
  • Deliver quality, clinical services to patients in accordance with MGB Healthcare at Home policies and standards of practice.
  • Collaborate with an interdisciplinary team of healthcare providers to ensure comprehensive patient care.
  • Document all patient interactions, treatments, and outcomes in a timely and accurate manner.
  • Develop and implement individualized care plans to meet the unique needs of each patient.

Requirements:

  • Graduate of an approved School of Nursing
  • Current Massachusetts license as Registered Nurse
  • Minimum of one year previous nursing experience in acute care setting; prior home care experience preferred
  • Travel required within MGB Healthcare at Home geographic area
  • Ability to work with various computer software and knowledge of hand-held patient care computer devices preferred
  • Strong interpersonal and customer service skills and ability to work with a diverse population
  • Ability to communicate effectively in writing, verbally, and electronically
  • Ability to work as a member of an interdisciplinary team of healthcare providers
  • Ability to organize and prioritize work and adapt to changing situations
  • Ability to work independently, be self-directed, and adapt to unpredictable circumstances
