Job Title: Per Diem Nurse

Mass General Brigham is seeking a skilled Per Diem Nurse to join our Home Health team. As a Per Diem Nurse, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence and improving health outcomes.

Job Summary:

We are looking for a compassionate and experienced Registered Nurse to work on a per diem basis in our Home Health department. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop plans to address these needs.
  • Provide education and support to patients and their families, promoting self-care and independence.
  • Document patient care and progress in a timely and accurate manner, using electronic health records.
  • Participate in multidisciplinary team meetings to discuss patient care and develop plans to address complex issues.
Requirements:
  • Current Massachusetts license as a Registered Nurse.
  • Minimum of one year of recent nursing experience in an acute or emergency setting.
  • Home health experience preferred.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and adapt to changing situations.
