- Gather and document information in the social service assessment and discharge plan.
- Create safe and healthy discharge plans that meet the needs of patients.
- Coordinate specialized aftercare services to ensure a smooth transition for patients.
- Serve as a point of contact for patients' wellbeing with members not on the care team.
- Act as the key informational flow regulator regarding a patients' progress in treatment.
