MDS Assessor

We are seeking a skilled and experienced MDS Assessor to join our team at The Hamlet Rehabilitation and Healthcare Center at Nesconset. As an MDS Assessor, you will play a critical role in ensuring the highest quality of care for our residents.

• Conduct thorough and accurate assessments of residents' health status and care needs, in accordance with federal and state regulations and company policies.
• Act as an in-house case manager, coordinating services with physicians, families, third-party payers, and facility staff to ensure seamless care delivery.
• Develop and implement effective care plans that address residents' unique needs and promote optimal outcomes.
• Collaborate with interdisciplinary teams to identify and address regulatory risks, clinical trends, and operational challenges.
• Ensure timely and accurate completion of Minimum Data Sets, Care Area Assessments, and Care Plans.
• Facilitate the care management process, engaging residents, families, and IDT members in timely identification and resolution of barriers to discharge.
• Provide education and training to IDT members on RAI processes, MDS Coordinator and Assessor roles, and company policies.
• Analyze QI/QM data to identify trends and inform quality improvement initiatives.
• Participate in various committees and meetings to ensure alignment with company goals and objectives.

• Registered Nurse with current, active license in state of practice.
• Minimum two years of clinical experience in a healthcare setting.
• Minimum of one year of experience in a long-term care setting.
• Prior experience as an MDS Coordinator or Assessor is preferred.
