ResponsibilitiesKey Responsibilities:
- Provide outpatient behavioral health services to adults with behavioral health and substance use disorders.
- Conduct individual and group therapy sessions to support patients' mental health and well-being.
- Perform home visits for supported living to ensure patients receive comprehensive care.
- Manage a caseload of 6-8 patients per shift, providing individualized attention and support.
- Document patient care accurately using the CareLogic electronic medical record (EMR) system.
Requirements:
- Specialty: LCSW or LPC (Licensed Clinical Social Worker or Licensed Professional Counselor)
- Minimum of 2 years of experience in clinical therapy (individual and group)
- Basic Life Support (BLS) certification
- Any state license accepted
Facility Details: Outpatient behavioral health integrated care
Patient Population: Adults with behavioral health and substance use disorders
Staff Mix: MDs, RNs, and a Behavioral Health Director
