We are seeking a skilled Physical Therapist to join our team at Medical Center Home Health, a part of LHC Group. As a Physical Therapist, you will play a vital role in helping patients achieve their maximum potential and improve their overall quality of life.

About the Role:

The Physical Therapist will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist will develop and implement a treatment plan to improve and enhance the patient's well-being.

Responsibilities:
  • Evaluate patient's functional status and physical therapy needs
  • Assist physician in developing therapy plan of care
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice
  • Instruct and inform participating members of the healthcare team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals
Qualifications:
  • Current Physical Therapy licensure in state of practice
  • Current CPR certification required
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
