Practice Environment:
This collaborative outpatient setting includes a diverse team of healthcare professionals, such as Family Physicians, Nurse Practitioners, Registered Nurses, Pediatricians, OB-GYNs, Dentists, and various specialists including optometrists, podiatrists, and dermatologists. We also have LCSWs, mental health experts, case managers, health educators, registered dietitians, and LVNs working together to provide comprehensive care.
Qualifications:
We strongly prefer candidates who are bilingual in English and Spanish to effectively serve our community. The role involves delivering high-quality primary care and managing chronic diseases, ensuring that all patients receive the attention they deserve.
Responsibilities:
- Conduct assessments, diagnoses, and management of patient healthcare needs using standardized and evidence-based practices.
- Provide primary care services focusing on chronic disease management.
Technology:
Experience with EPIC EMR is preferred.
Work Schedule:
- Full-time position available: 4 or 5 days per week.
- Standard hours are Monday to Friday, 8 AM to 5 PM, with one Saturday per month required, for which a weekday will be adjusted.
Compensation and Benefits:
- Competitive salary range: $220,000 - $265,000 annually, depending on experience.
