POSITION OVERVIEW:
The role involves delivering pharmaceutical services for the organization and patients through the meticulous assessment of physician directives, compounding of sterile formulations, and secure distribution of medications.
The clinical pharmacist offers pharmaceutical care, guidance, and/or consultation to patients and healthcare providers.This position may oversee the functions of pharmacy support staff and engage in quality assurance and performance enhancement initiatives.
EDUCATIONAL AND PROFESSIONAL REQUIREMENTS:
Must hold a valid Registered Pharmacist license in good standing with the State Board of Pharmacy.
Preferred: Minimum one year of experience in intrathecal therapy or high-risk sterile compounding.
KEY RESPONSIBILITIES:
Upholds patients' rights and responsibilities while supporting them in the execution of job functions; honors the patient's right to privacy and confidentiality.
Compounds patient-specific sterile formulations for dispensing utilizing aseptic techniques.Reviews, processes, and verifies physician orders and patient medication records.
Maintains up-to-date knowledge of USP regulations to ensure the quality, sterility, and stability of compounded sterile formulations.
Utilizes available resources to collect information on pharmaceuticals, equipment, and patient monitoring.
Assists in the upkeep and sanitation of pharmacy workspaces and the execution of quality assurance and performance improvement protocols as directed by the pharmacy manager.
Confirms syringe orders and compounds according to physicians' specifications.Prepares sterile medications for patient administration, ensuring precision and sterility.
Processes prescriptions with thorough quality checks to guarantee appropriateness and accuracy.
Collaborates with other team members/departments to facilitate workflow and assist in inventory management.
Follows up with prescribers regarding incomplete information on deficient prescriptions.
Adheres to proper garbing protocols before entering the compounding area, which includes full coveralls with integrated hoods, goggles, gloves, shoe covers, and facemasks to ensure complete skin coverage.Participates in initial training and competency assessments, as well as ongoing education related to job responsibilities.
Serves as a resource and consultant to physicians, referral sources, and patients regarding potential adverse effects and interactions associated with dispensed medications.
Acts as the patient care coordinator with external nursing agencies; collaborates with nursing staff regarding patient care, medication delivery, and clinical evaluations.
Directly supervises pharmacy support staff in the correct compounding, packaging, and labeling of sterile products.Maintains accurate documentation of care provided to patients and of drug prescriptions and compounding as outlined in the Pharmacy Policy and Procedure Manual and as required by state regulations.
Assists with the orientation and training of new employees as necessary.Completes required documentation in a timely manner as directed.
Shares knowledge with colleagues and fosters teamwork.
Interacts with others in a positive, respectful, and considerate manner.
Performs additional job-related duties as assigned.
QUALIFICATION CRITERIA:
Understanding of laminar airflow principles and knowledge of drug stability and incompatibility.
Must fully comply with garbing requirements outlined in the essential duties and responsibilities section as well as in company policies and procedures for extended periods.
Ability to communicate effectively and professionally with colleagues, referral sources, physicians, and patients.Informed about current clinical issues, industry standards, and accreditation requirements, with the ability to integrate these standards into daily practice.
Demonstrates exceptional attention to detail and accuracy.Ability to prioritize tasks and meet deadlines.
Proficient in mathematics.
Strong analytical capabilities.
Familiarity with equipment such as repeater pumps, analytical balances, and hot plates.
Exhibits good personal hygiene.
Must adhere to all personal hygiene and gowning requirements as specified in USP.
Basic computer skills and familiarity with Microsoft applications.
