About the RoleAs a Primary Care Physician at CenterWell, you will be responsible for evaluating and treating patients in accordance with our standards of care. You will work closely with our interdisciplinary care team to coordinate patient services, including specialty referrals, hospital and SNF coordination, and durable medical equipment and home health care.
Responsibilities- Evaluate and treat patients in accordance with our standards of care
- Coordinate patient services, including specialty referrals and hospital coordination
- Assist in the development and implementation of care plans
- Participate in daily huddles with the care team to discuss patient care and coordinate services
- Complete all medical record documentation in a timely manner
- Meet with the Regional Medical Director to discuss quality of care, review outcome data, and address policy and procedure issues
- Current and unrestricted medical license
- Graduate of an accredited MD or DO program
- Excellent verbal and written communication skills
- Demonstrate a high level of skill with interpersonal relationships and communications with colleagues and patients
- Fully engaged in the concept of integrated team-based care
- Willingness and ability to learn and adapt to practice in a value-based care setting
- Board Certification or Eligible to become certified (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine
- Bilingual English/Spanish is preferred
- Active and unrestricted DEA license
- Medicare Provider Number
- Medicaid Provider Number
- Minimum of two to five years directly applicable experience
CenterWell Senior Primary Care provides proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, minor injury treatment, and more.
