We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Case Manager. As a key member of our interprofessional team, you will provide end-of-life care and education to patients and their families with empathy and compassion.
Responsibilities- Evaluate patient preferences and barriers to involvement in care, including their values, emotional, spiritual, cultural, and population-specific needs.
- Develop, implement, and document individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient, family or caregiver.
- Proactively plan and ensure communication of the plan of care across the continuum of care.
- Promote respect, equity, and empathy in interactions with diverse and vulnerable populations through care delivery.
- Practice collaborative problem-solving, service recovery, and advocacy for patient-family-centered continuity of care.
- Implement care by integrating data from the interprofessional team and critical thinking in a safe and timely manner.
- Evaluate changes in patient's condition, inform and collaborate with family and/or caregivers, and communicate with interprofessional team as changes occur in plan of care.
