Job Summary

Boston Medical Center Health System is seeking a highly skilled Nurse Practitioner to join our Community Health Equity team. The successful candidate will be responsible for providing high-quality, patient-centered care to individuals with cardiometabolic conditions, working in partnership with cardiologists and endocrinologists.

The ideal candidate will possess a Master's degree in nursing, current licensure to practice as a registered nurse in the Commonwealth of Massachusetts, and authorization by the Board of Registration in Nursing to practice in an expanded role. A minimum of three years of current clinical nursing experience and two years of experience in adult practice are required.

The successful candidate will be responsible for overseeing the initiation, titration, and optimization of guideline-directed medical therapy, leading weekly care consultations with cardiologists and endocrinologists, and partnering with community health workers to ensure timely engagement and navigation of patient health-related social needs.
