Home Health Physical Therapist Job Opportunity

LHC Group is seeking a skilled Physical Therapist to join our team in Parkersburg, West Virginia. As a Home Health Physical Therapist, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.

Key Responsibilities:
  • Evaluate patient's functional status and physical therapy needs
  • Assist physician in developing therapy plan of care
  • Instruct and inform participating members of healthcare team, patient, and family/caregivers regarding plan of care, functional limitations, and progress toward goals
Requirements:
  • Current Physical Therapy licensure in state of West Virginia
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
