Hospice Social Work Professional

This role involves providing skilled social work services to patients facing life-limiting illnesses. The ideal candidate will possess excellent communication skills, a strong understanding of psychosocial factors influencing terminally ill patients, and experience working within interdisciplinary teams.
Key Responsibilities:
* Conduct thorough psychosocial assessments to inform care planning
* Develop and implement individualized care plans, collaborating with healthcare professionals
* Foster connections between patients/families and community resources
* Maintain accurate clinical records and document progress
Essential Qualifications:
* MSW or related graduate degree from an accredited institution
* Active licensure as a LSW or equivalent
* Proven ability to communicate effectively with diverse populations
* Familiarity with hospice care principles and practices
Salary Information:
Annual salary range: $80,000 - $100,000
