Key Responsibilities

  • Collaborate with healthcare staff to deliver patient care effectively, utilizing evidence-based practices and research to inform care decisions.
  • Create a safe and caring environment for patients, families, and staff, promoting emotional and physical well-being.
  • Build relationships with patients, families, and staff, focusing on managing emotional and physical pain to promote healing and reduce suffering.
  • Facilitate learning for all members of the healthcare team, ensuring everyone is well-informed and empowered to provide high-quality care.
  • Utilize problem-solving strategies and available resources to support patients, families, and staff, taking a systems-thinking approach to care delivery.
Requirements
  • Completion of an accredited course in nursing, with a focus on hospice care and end of life care.
  • A minimum of 2 years of experience in a Home Health or Hospice RN role, with a strong background in patient care and management.
  • Certification in Basic Life Support (BLS) is required, with the ability to obtain certification within 30 days of starting in this role.
  • Registered Nurse License is required, with a strong understanding of nursing practice and patient care standards.
