We have been acknowledged by both regional and system leadership for our innovative approach to decreasing readmissions among high-risk patients lacking consistent primary care.
Work Schedule:
16 hours per week.
Key Responsibilities:
- Exhibit autonomy in evaluating and managing chronic health conditions, utilizing critical thinking to determine the best course of action for patient clinical optimization.
- Oversee the surgery evaluation clinic (approximately 60-70% of your responsibilities) alongside the transitional care clinic.
- Conduct thorough assessments for patients of all ages, gathering comprehensive medical histories and performing detailed physical examinations.
- Prescribe suitable pharmacological and non-pharmacological treatment options.
- Order and interpret diagnostic and therapeutic tests tailored to the specific needs of patients across various age groups.
- Respond promptly to inquiries from patients and staff, prioritizing urgent needs and ensuring effective communication.
- Develop and implement comprehensive care plans based on advanced diagnostic skills and professional judgment, reviewing test results and coordinating treatment strategies for patients and their families.
- Demonstrate problem-solving abilities by making informed clinical decisions, utilizing available resources, and consulting with physicians as necessary.
- Maintain open lines of communication with staff to ensure that information and administrative decisions are shared positively, fostering a supportive work environment.
- Monitor the effectiveness of interventions and adjust care plans as needed.
- Facilitate patient transitions within and between healthcare settings, including admissions, transfers, and discharges.
- Collaborate with multidisciplinary team members, making appropriate referrals to community healthcare facilities and resources.
- Support decision-making for staff, patients, and families by providing educational resources.
- Act as a change agent by introducing innovative nursing practices that enhance patient care.
- Evaluate nursing practices in specialty areas, identifying needs and proposing constructive solutions to improve patient outcomes.
- Encourage positive health behaviors and self-care through education and counseling.
- Maintain a vigilant and safe working environment.
- Perform additional duties as assigned.
Education Requirements:
- Master's degree from an accredited Nurse Practitioner or Physician Assistant program.
Experience Requirements:
- Minimum of one year of relevant experience.
Physical Requirements:
- Frequent verbal communication and interaction with patients and staff.
- Regular standing and walking throughout the workday.
- Occasional lifting and carrying of objects weighing up to 50 lbs.
- Frequent use of vision for close and distant tasks, as well as color differentiation.
- Ability to detect odors and respond to auditory signals.
- Occasional driving and mobility-related tasks.
