Position Overview
This role involves collaborating with the Interdisciplinary Team (IDT) to formulate and execute a comprehensive care plan for patients. Responsibilities include:
1. **Developing Care Plans:** Working alongside the IDT to create tailored care strategies.
2. **Implementing Care:** Carrying out the established care plans effectively.
3. **Obtaining Orders:** Securing physician's orders as required.
4. **Specialized Nursing Services:** Delivering nursing services that necessitate advanced skills.
5. **Patient and Family Education:** Instructing patients and their families on care procedures, including pain management techniques.
6. **Outcome Evaluation:** Assessing the effectiveness of provided services.
7. **Communication:** Coordinating and conveying the patient's medical status to the IDT.
8. **Documentation:** Preparing clinical notes and overseeing the work of licensed practical nurses and home health aides in delivering delegated nursing services.
9. **Plan Coordination:** Ensuring the effective implementation of each patient's care plan.
Additional Responsibilities:
This position may involve cross-training within the agency, which could require access to medical records and other computer systems beyond standard usage.
Qualifications:
- Associate's or Bachelor's degree in Nursing from an accredited institution.
- Minimum of two years of nursing experience, preferably in Med/Surg, critical care, or adult/geriatric nursing.
- Valid and active RN licensure from the appropriate state.
- CPR certification is mandatory.
- Must possess a current driver's license and maintain liability/collision insurance.
