We are seeking a dedicated and empathetic Home Health Nurse Specialist to join our team at Human Touch Home Health Care.

About the Role:

  • Provide comprehensive nursing care to patients in their homes, focusing on preventive, rehabilitative, and therapeutic interventions.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans, addressing physical, emotional, and social needs.
  • Conduct regular evaluations, making necessary revisions to care plans as patient status and needs change.
  • Administer medications and treatments as prescribed by physicians, ensuring safe and effective care delivery.
  • Act as Case Manager when assigned, coordinating patient care for assigned caseloads.

Requirements:

  • Graduate of an accredited school of nursing, with 1-2 years of acute care experience in an institutional setting.
  • Current licensure in state and CPR certification.
  • Bachelor's degree, with 1 year of home health care experience preferred.
  • Excellent observation, verbal, and written communication skills; problem-solving skills; basic math skills; and nursing skills per competency checklist.
