About the Role

As a Speech Language Pathologist at CenterWell Home Health, you will be part of a multidisciplinary team providing comprehensive home care services to patients with communication disorders. Your expertise will play a vital role in helping patients manage their conditions and achieve improved quality of life.

Responsibilities

The successful candidate will be responsible for:

  1. Evaluating patients with communication disorders and developing individualized plans of care
  2. Providing speech and language therapy services to patients in the home or facility
  3. Collaborating with other healthcare professionals to ensure coordinated care and optimal outcomes
  4. Maintaining accurate records of patient progress and communicating effectively with patients, families, and healthcare teams

Requirements

To be successful in this role, you will need:

  • Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
  • Minimum of six months experience as a speech therapist/speech language pathologist
  • Current and unrestricted license
  • Current CPR certification
