The Opportunity

We're seeking a skilled Health Coach to join our team at our Hyannis, MA office. As a Health Coach, you'll be an integral part of our primary care team, working closely with patients and their families to support their preventive, chronic, and acute care needs. Your primary responsibilities will include establishing trusting relationships with patients, advocating for their needs, and collaborating with our integrated primary care team to ensure seamless care.

Key Responsibilities:
  • Capture relevant patient information while rooming patients, taking vitals, and performing standard assessments
  • Assist with post-visit care coordination, scheduling appointments, and sharing information with the patient's care team
  • Collaborate with providers to monitor patient health and ensure preventive measures are up to date
  • Participate in daily practice operations, including answering phone calls, responding to emails, and maintaining the clinical space
  • Provide patient education on chronic disease management and coach patients using motivational interviewing techniques
Requirements:
  • At least 1 year of experience in a high-touch customer service or patient-facing role in a healthcare setting
  • Strong written and verbal communication skills
  • Experience working on collaborative, diverse, and feedback-driven teams
  • A proven track record of persisting through change and consistently stepping up to take action on challenges
  • Proficiency in computer technology, including typing, navigating the internet, and using multiple software systems simultaneously
