Nursing Case Manager

About the Position:
We are seeking a skilled and compassionate RN Case Manager to join our team at Professional Case Management. As an RN Case Manager, you will be responsible for providing in-home nursing care and case management to former Nuclear Weapons Workers suffering from chronic and terminal illnesses.
Your Key Responsibilities:
- Conduct in-home nursing visits for clients, including physical assessments, ensuring home safety, and managing care.
- Collaborate with regional directors and other personnel to identify and correct issues or improve services.
- Plan, implement, and evaluate care provided to clients.
- Serve as a local on-site clinical resource and provide support to ensure clients' home care needs are met.
Requirements:
- Graduate of a state-approved school of professional registered nursing.
- Current, unrestricted RN license in the state(s) of practice.
- Minimum of two years of nursing experience, including one year in home care or a closely related field.
- One year of supervisory and/or case management experience preferred.
- Current CPR certification.
Benefits:
- Competitive pay rate of $42.00 hourly, plus shift differentials where applicable.
