Job Summary

The Private Duty Nurse LVN will provide one-on-one care to patients in their homes, assisting with daily living activities and administering medications as prescribed. This is a rewarding opportunity to make a positive impact on the lives of our patients and their families.

Responsibilities
  • Implement physicians' prescribed plans of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medications, treatments, and therapies
  • Document care provided in our system of record
  • Teach and train family members on health promotion and disease prevention
  • Accept and write verbal orders from physicians as required
  • Follow up with physicians, clients, family members, and staff on client problems and concerns
Requirements
  • Active, unencumbered LVN license in the state of practice
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred
  • Reliable transportation
