Your Role
As a Practical Nursing Specialist, you will collaborate closely with families and psychiatry services staff to ensure the delivery of high-quality, personalized integrated healthcare within the CCBHC framework.


Key Qualifications:
- Active LPN license in the state of Minnesota.
- Current BLS/CPR Certification is mandatory.
- Preferred experience working with pediatric populations facing behavioral health challenges.
- If driving is required, you must possess a valid Minnesota driver's license and maintain a satisfactory driving record in accordance with Fraser policy.
- Access to a reliable vehicle in good working condition.

Core Responsibilities:
- Deliver exceptional mental health client care as part of a collaborative healthcare team within the CCBHC model.
- Coordinate client management alongside members of Fraser Psychiatry Services and the Fraser Care Team.
- Assist Fraser staff in gathering information to evaluate risk for incoming and ongoing clients of Fraser Psychiatry Services.
- Provide enhanced support for high-risk patients, acting as a liaison with primary care providers and other team members as necessary.
- Arrange essential support services in line with risk levels and care plans.
- Monitor issues, address barriers to care, and implement strategies to reduce emergency department utilization among high-risk clients.
- Participate in scheduling, rooming, referrals, prescriptions, and follow-ups for psychiatric and internal/external services.
- Support the completion of diagnostic evaluations, medication therapies, and medical management tasks as requested by Fraser providers.
- Maintain thorough and accurate documentation of client interactions and keep clinical files up to date.
- Communicate effectively with other providers, sharing information about Fraser Team Care as it relates to clients.
- Request initial and refill prescriptions.
- Seek training opportunities to stay informed about best practices.
