As a Nurse Care Specialist, you will provide expert nursing care to patients in post-acute care facilities, working closely with residents and medical staff to develop care plans and ensure the highest level of patient care.
Responsibilities- Provide expert nursing care to patients in post-acute care facilities
- Develop and implement individualized care plans in collaboration with residents and medical staff
- Monitor and track patient outcomes, making adjustments to care plans as needed
- Collaborate with medical staff to ensure accurate and timely documentation of patient histories and interventions
- Supervise and train nursing staff, ensuring compliance with facility policies and nursing standards
- Participate in incident investigations and provide recommendations for improvement
- Current Connecticut state license to practice as a Registered Nurse (RN)
- Graduation from an approved nursing program
- At least 1 year of experience as an RN or in a similar role
- Excellent organizational and leadership skills
- Competitive hourly pay rate: $50-$82
