Job Summary

We are seeking a skilled Nurse Case Manager to join our team at Albany Medical Center. As a key member of our interdisciplinary team, you will be responsible for coordinating patient care across the continuum of care, ensuring timely and appropriate interventions to achieve optimal patient outcomes.

Key Responsibilities

  • Collaborate with the interdisciplinary team to plan, coordinate, and implement patient care plans.
  • Monitor patients' clinical progress and ensure timely interventions to achieve optimal outcomes.
  • Provide collaborative care management with primary nurses to assess discharge planning needs and coordinate resources.
  • Collaborate with the healthcare team to manage care across the continuum, including pre-admission, discharge, and post-discharge planning.
  • Utilize evidence-based guidelines to provide leadership and guidance to the healthcare team in formulating individualized multidisciplinary plans of care.

Requirements

  • Bachelor's Degree in Nursing required; Master's Degree preferred.
  • 1-3 years of clinical experience in an assigned service required.
  • Recent experience in case management, utilization management, and/or discharge planning/home care in a high-volume, acute care hospital preferred.
  • Effective communication, facilitation, and organizational skills required.
  • Assertive and creative problem-solving, critical thinking skills, systems planning, and patient care management required.
  • Self-directed with the ability to adapt in a changing environment required.
  • BASIC knowledge of computer systems with skills applicable to utilization review process required.
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure Upon Hire required.
  • Certified Case Manager and PRI Upon Hire preferred.
