Alternate Solutions Health Network LLC is seeking a skilled Occupational Therapist to join our team in Detroit, Michigan.

Job Summary:

We are looking for a compassionate and detail-oriented Occupational Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Occupational Therapist, you will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote patient independence and well-being.

Responsibilities:
  • Provide skilled occupational therapy services to patients in their homes, as ordered by the attending physician.
  • Evaluate patient's level of functioning and develop individualized plans of care to improve range of motion, muscle strength, and coordination.
  • Teach patients and their families on proper use of equipment and techniques to promote independence and safety.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and coordinate services.
  • Document patient progress and outcomes in accordance with agency protocol and Medicare/Federal guidelines.
Requirements:
  • Current license as an Occupational Therapist in the state of employment.
  • Minimum one year of experience as an OT in an acute care setting, with home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
  • Ability to meet the physical demands of the job.
