Job Summary

The Clinical Patient Care Manager Home Care is a key member of the Patient Care Team, responsible for leading and coordinating the delivery of high-quality patient care services. This role requires strong leadership and communication skills, as well as the ability to work collaboratively with interdisciplinary teams to ensure seamless care transitions.

Key Responsibilities
  • Lead and coordinate patient care services, ensuring timely and effective delivery of care
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans
  • Monitor and evaluate patient outcomes, making adjustments to care plans as needed
  • Supervise and mentor staff, providing guidance and support to ensure high-quality care delivery
  • Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care
Requirements
  • Current and valid RN license to practice in the state where the VITAS program is located
  • Two years of successful supervisory experience in a healthcare organization
  • Knowledge of hospice care principles and practices, as well as oncologic nursing and palliative care
  • Strong leadership and communication skills, with the ability to work collaboratively with interdisciplinary teams
