Job Summary:
We are seeking a highly skilled and compassionate Registered Nurse/Licensed Practical Nurse to join our team at Aspen Transitional Rehab. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and support to our patients in a subacute rehabilitation setting.
Responsibilities:
- Provide direct patient care and support to patients in a subacute rehabilitation setting
- Supervise and coordinate the care of Certified Nursing Assistants (CNAs) to ensure high-quality patient care
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes
- Participate in ongoing education and training to stay current with best practices and industry developments
Requirements:
- Current licensure as a Registered Nurse or Licensed Practical Nurse in the state of Idaho
- At least 6 months of experience in long-term care or rehabilitation
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Strong problem-solving and critical thinking skills

Aspen Transitional Rehab is a 30-bed inpatient subacute rehabilitation facility located in Meridian, ID. Our state-of-the-art facility offers a supportive and collaborative work environment, with a focus on providing exceptional patient care and outcomes.
