We are seeking a highly skilled Home Health Physical Therapist to join our team in West Virginia. As a Physical Therapist with LHC Group, you will have the opportunity to make a meaningful impact on the lives of our patients and their families.

About the Role

The Physical Therapist (PT) is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the PT works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.

  • Evaluates the patient's functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
  • Observes and reports the patient's response to treatment and any changes in his/her condition to the supervising nurse and/or the physician.
  • Treats patients to relieve pain, develop and restore function, and / or maintain maximum functioning using acceptable standards of practice.
  • Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.

Requirements and Qualifications

To be considered for this role, applicants must have current Physical Therapy licensure in the state of West Virginia, current CPR certification, and a current driver's license and vehicle insurance. A reliable means of transportation is also required.

Compensation and Benefits

We offer a competitive salary of up to $120,000 per year, based on full-time employment and maximum productivity. Additionally, we provide relocation assistance of $5,000 for those who qualify for full-time employment. Our comprehensive benefits package includes holiday pay, medical, dental, vision benefits, 401k retirement plan, employee assistance programs, and more.
