Job Summary

The LPN will be responsible for providing skilled nursing care to patients in their homes, under the supervision of a Registered Nurse. This role requires strong clinical skills, excellent communication, and a commitment to delivering exceptional patient care.

Key Responsibilities
  • Deliver and coordinate quality patient care in compliance with physician orders
  • Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner
  • Participate in and update the nursing care plan
  • Take appropriate nursing action based on assessment and achieve expected outcomes
Requirements
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
