About the Role
As a Case Management Registered Nurse PRN, you will play a vital role in ensuring high-quality, patient-centered care by managing rehabilitative services to achieve optimal outcomes. You will coordinate and direct care tailored to the unique needs of each rehabilitation patient.
Key Responsibilities:
- Provide orientation to patients, families, and caregivers regarding the case manager's role, rehabilitation philosophy, and discharge criteria.
- Facilitate patient and family education, encouraging their participation in discussions about care plans and goals.
- Implement treatment plans that reflect the patient's strengths and preferences, ensuring their involvement throughout the rehabilitation process.
- Document findings from evaluations and assessments, communicating the needs and preferences of patients and families.
- Promote consistent, positive interactions with patients to enhance their experience and care.
Qualifications:
Required Experience:
- A minimum of 3 years in nursing practice, with clinical knowledge and effective communication skills.
Preferred Experience:
- 5 years in nursing practice and 3 years in care management, with experience in community or ambulatory care management.
Education:
- Bachelor of Science in Nursing or a related field is required. Candidates must maintain current licensure as a registered nurse.
Certifications:
- BLS certification is required. Nationally recognized certification in Case Management is preferred.
