Primary Care Physician

ChenMed is seeking a highly skilled and compassionate Primary Care Physician to join our team. As a Primary Care Physician, you will play a key role in providing high-quality, patient-centered care to our senior patients.
Responsibilities:- Provide comprehensive primary care to patients, including medical history, physical exams, diagnosis, and treatment.
- Develop and implement patient management plans, including referrals to specialists and coordination of care.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Participate in quality improvement initiatives and contribute to the development of clinical policies and procedures.
- Stay up-to-date with the latest medical research and best practices to provide evidence-based care.
- Current licensure in the state of practice.
- Certification in Basic Cardiac Life Support.
- Board certification in Internal Medicine or Family Medicine.
- Minimum of one year of clinical experience in geriatric, adult, or family practice setting.
