Job Summary

We are seeking a skilled Nurse Practitioner to join our team in providing high-quality primary care to geriatric patients in skilled nursing facilities, assisted living facilities, independent living facilities, and private homes.

Key Responsibilities
  • Conduct health assessments to identify patient needs and develop personalized care plans
  • Regularly evaluate patient conditions and adjust treatment plans as necessary
  • Administer medications, treatments, and other therapeutic interventions as prescribed
  • Perform minor procedures and wound care as required
  • Monitor vital signs and patient responses to treatment
  • Provide guidance on lifestyle modifications and self-care practices
  • Work with physicians, nurses, and other professionals to coordinate comprehensive care
Requirements
  • Ohio NP license
  • DEA
  • 1+ year advanced practice experience
  • Previous experience in geriatrics, LTC, SNFs, house calls, etc.
