Role Overview

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Healthcare Professional - Home Care Specialist. As an LPN at Optimae, you will be responsible for implementing care plans, conducting patient assessments, administering medications, and educating patients and their caregivers on health conditions and care plans.

Responsibilities:

  • Care Plan Implementation: Develop and implement individualized care plans that address the unique needs of each patient.
  • Patient Assessments: Conduct thorough assessments of patients' physical and emotional well-being to inform care planning and ensure optimal outcomes.
  • Medication Administration: Administer medications and treatments as prescribed by physicians, ensuring accuracy and adherence to safety protocols.
  • Patient Education: Educate patients and their caregivers on health conditions, care plans, and self-care techniques to promote independence and confidence.
  • Collaboration: Work collaboratively with RNs, physicians, and other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Documentation: Maintain accurate and thorough documentation of patient interactions, care plans, and progress notes to support quality improvement initiatives.

Qualifications:

  • A valid LPN license in the state of Iowa.
  • Reliable transportation and a valid driver's license.
  • Strong communication and organizational skills.
  • Ability to work both independently and collaboratively.
  • A passion for providing home care and supporting mental/behavioral health needs.

Compensation and Benefits:

  • A competitive salary range of $65,000 - $85,000 per year.
  • Mileage reimbursement at 44 cents per mile.
