Responsibilities- Provide skilled pediatric care to patients in their homes, including medication management, wound care, and vital sign monitoring.
- Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to develop and implement individualized care plans.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
