Job Summary:

We are seeking an experienced and highly motivated individual to join our team as a Case Manager 3 in the Division of Developmental Disabilities. The successful candidate will provide services and supports to individuals with developmental disabilities and their families, promoting independence and community engagement.

Key Responsibilities:

  • Conduct on-site visits to develop relationships and discuss service needs and supports with members and families.
  • Develop, write, review, monitor, and evaluate Individual Support Plans (ISPs) in accordance with mandated timelines.
  • Research community resources to meet identified needs and inform members of available options.
  • Facilitate meetings and program visits to assist with member and family choices, and complete referrals for assessed and covered services.
  • Collaborate with community agencies to provide assistance and support to members and families in crisis.
  • Mentor and train new and existing team members in the unit.

Requirements:

  • A Bachelor's degree in Social Work, Psychology, Special Education, or Counseling, or a related field, and at least one year of experience providing case management services to individuals with developmental disabilities.
  • Or, a minimum of two years' experience providing case management services to individuals with developmental disabilities.
  • A valid Arizona driver's license and a clean driving record.
  • Arizona Level 1 Clearance Card.
